Required Health Examinations

Required Health Exams

Required health examinations (physical and dental) are due by September 30th of the current school year.  Parents are encouraged to communicate with the building certified school nurse regarding a plan to meet the requirement.

The following health exams are mandated by the Pennsylvania Department of Health: 

Physical Exams

  • A physical examination is required upon original entry into school (Kindergarten/1st) and again in 6th and 11th grades.
  • It is recommended that this exam be performed by your private physician who is more familiar with your child’s medical history.
  • The required private physical examination form can be obtained from your child’s school nurse or downloaded via the following link: Private or School Physical Examination of School Age Student. The private examination may be done within one year from the start of the current school year. Once the private examination is completed, return the private physical form to your school nurse.
  • If you choose to have the physical examination done by the school physician, you will be notified of the specific date and time by the school nurse.  

Dental Exams

  • A dental examination is required upon original entry into school (Kindergarten/1st) and again in 3rd and 7th grades.
  • The required private dental examination form can be obtained from your child’s school nurse or downloaded via the following link: Private Dentist Report of Dental Examination of a Pupil of School Age. The private examination may be done within one year from the start of the current school year. Once the private examination is completed, return the private dental form to your school nurse.
  • If you choose to have the dental examination done by the school dentist, you will be notified of the specific date and time by the school nurse.
