Overnight Field Trip Checklist
In order to ensure the health and wellness of students attending overnight field trips, the following documents are required:
7th grade camping trip:
□ Documentation of required 6th grade Physical Examination & Immunizations
□ Documentation of required 7th grade Dental Examination – must be submitted by Sept 30th
□ Completed and signed Standing Orders / Health Updates (dated after July 1st of the current school year)
□ Notification that medication will need to be administered during the overnight field trip
8th grade Washington DC field trip:
□ Documentation of required 6th grade Physical Examination & Immunizations
□ Documentation of required 7th grade Dental Examination
□ Completed and signed Standing Orders / Health Updates (dated after July 1st of the current school year)
□ Notification that medication will need to be administered during the overnight field trip
10th grade Boston field trip:
□ Documentation of required 6th grade Physical Examination & immunizations
□ Documentation of required 7th grade Dental Examination
□ Completed and signed Standing Orders / Health Updates (dated after July 1st of the current school year)
□ Notification that medication will need to be administered during the overnight field trip
12th grade overnight field trip:
□ Documentation of required 11th grade Physical Examination & immunizations
□ Documentation of required 7th grade Dental Examination
□ Completed and signed Standing Orders / Health Updates (dated after July 1st of the current school year)
□ Notification that medication will need to be administered during the overnight field trip
Should your child require medication on an overnight field trip, the following documents are required:
□ Completed Authorization for Medication During Overnight Field Trips form.
□ Completed Action Plan for Allergy, Asthma, Diabetes and/or Seizure.
□ Completed Emergency Action Plan for Allergy, Asthma, Diabetes and/or Seizure.
Authorization for Medication During Overnight Field Trip Procedures
Medication, including prescription and over-the-counter medications, herbal or homeopathic supplements, oils and/or vitamins should not be administered on field trips unless absolutely necessary. If it is crucial for your child to take medication on a field trip, please refer to the following procedures/guidelines:
1. The medication must be submitted to the school nurse with the completed Authorization for Medication During Overnight Field Trips form.
2. The Authorization for Medication During Overnight Field Trips form must state the following:
- Necessity of the medication (diagnosis)
- Name of the medication
- Medication dosage
- Time the medication is to be administered (frequency and expected duration)
- Side effects of the medication
3. The Authorization for Medication During Extended Field Trips form must be signed by both the student’s physician and the student’s parent/guardian.
4. All medication must be brought to the school by a parent/guardian. Medication may not be transported to and from school by the student.
5. Prescription medication must be supplied in the original container and properly labeled by a registered pharmacist or licensed physician. Non-prescription medication must be in the original container with the label intact.
6. The first dose of any new medication will not be given in school for your child’s safety.
7. Students are expected to report to the school nurse at the prescribed time of administration.
8. For any student requiring medication on a school-sponsored field trip, it is the parent/guardian’s responsibility to contact the school nurse to plan for proper administration of medication on the trip. Medication will not automatically be sent on field trips.
9. All student medication must be picked up by the student’s parent/guardian and will not be sent home with the student. Unused medication must be picked at arrival back to school after field trip.
